Click "Apply to Blog" and you're done.

If it's a new blog, it take a few days for Google bots to index and list your blog onto its search engine. If not, try submitting your blog link to Google Webmaster.

The template is originally like that. There is a way to "push up" the middle portion but you'll need some knowledge in HTML to do that.
